The UEA Benefits Program only collects personal information from you that is necessary for us to provide services to you and to otherwise support our business operations.
When you participate in our program, we will ask you for certain business or personal information (i.e. your name, business name, telephone number, address, email etc.). This type of information can be used to identify you as an individual or business. We receive this personally identifiable information directly from you or from certain unaffiliated third parties authorized to release personal and business information, including insurance companies, administrators, and service providers.
We maintain safeguards to protect your data. We review our privacy practices, monitor our computer networks and test the strength of our security measures. We require companies that provide services to us to agree to safeguard customer data. Only those members of our staff involved with administering the plan have access to your personal data. Except as stated above, we will not share your personally identifiable information outside the scope of our business operations without written permission from you except to: (1) comply with the law or respond to lawful requests or legal process, such as a court order, subpoena, search warrant or other legal requirements of any government authority; (2) protect the rights or property of the UEA Benefits Program; (3) enforce our agreements, policies and terms of use or to mitigate our liability in an actual or potential lawsuit; (4) respond to emergencies; and (5) protect against fraud or security hazards.
